Physical Therapist – In-Home Neurologic Rehab

SynapseTBI is a neurodiagnostic and rehabilitation company specializing in TBI, mTBI, and Post-Concussion Syndrome , delivering ACOEM-validated diagnostic and treatment protocols.
We’re hiring a licensed Physical Therapist to provide in-home neurologic and functional evaluations and treatment for brain injury patients. This role is ideal for PTs seeking flexible, independent, patient-focused work outside a traditional clinic.
Responsibilities:
• Perform in-home evaluations and treatment for TBI/post-concussion patients
• Develop and implement individualized rehab plans
• Collaborate with neurologists and multidisciplinary teams
• Document care clearly and efficiently
Qualifications:
• Active Physical Therapist license
• Neuro, vestibular, or post-concussion experience preferred
• Comfortable working independently in patient homes
• Reliable transportation
• Telehealth experience a plus
